Intake Specialist

Job Overview:

This position plays a crucial role in ensuring the smooth operation of our corporate office and maintaining high standards in patient care delivery. As an Intake Specialist, you will be responsible for processing patient referrals and inquiries efficiently, while interacting with patients, physicians, and other healthcare providers. This is a full-time position, Monday through Friday, from 8 AM to 5 PM, based in our corporate office.Each specialist is responsible for processing an equal number of referrals daily. You will receive comprehensive training and access to resources from our marketing team to become well-versed in specialized wound care modalities, allowing you to effectively represent Total Wound Care. Staying up to date on all processes and flow charts related to the intake process will be key to your success. Teamwork and collaboration are essential to ensure a seamless workflow and exceptional patient care.


Responsibilities:

Referral Management: Receive and input patient inquiries and referrals via phone, email, or in person into the referral tracker and EMR system.

Initial Assessments: Conduct initial assessments to determine patient needs and eligibility for services.

Insurance Clarification: Call and clarify UltraMIST application status and benefits.

Appointment Coordination: Coordinate patient appointments and schedule discussions with the appropriate clinical staff.

Records Management: Maintain accurate records of patient information and interactions in the database.

Collaboration: Work closely with internal teams to ensure seamless coordination of patient services.

Resource Provision: Provide information, resources, and referrals to patients, home health agencies, case managers, clinical staff, account executives, and assisted living or long-term care facilities.

Referral Progress Monitoring: Monitor and track patient progress and follow up as necessary.

HIPAA Compliance: Adhere to HIPAA regulations and maintain patient confidentiality at all times.

Additional Duties: Assist with managing the ancillary tracker, hospital tracker, and other tasks as needed.

Qualifications:

Education: High school diploma or equivalent is required.

Experience: Proven experience in customer service or a client intake role, preferably in a healthcare setting.

Software Proficiency: Proficiency in Microsoft Office and data management software.

Communication Skills: Strong communication and interpersonal skills.

Organizational Skills: Ability to work under pressure, manage multiple tasks, and stay organized.

Healthcare Knowledge: Familiarity with community resources and social services is beneficial.

Confidentiality: Ability to maintain confidentiality and handle sensitive information with discretion.
